Level I
Self Risk Assessment
no charge
Recommended for women of all ages, this self assessment will evaluate an individual's risk of cardiovascular disease. The more you know about your risks, the better able you'll be to control those risks factors. And it's simple: just download and fill in the assessment form and mail it to the address located at the bottom of the form. A Cardiovascular Nurse will review the questionnaire and follow up with you.
Level II
Cardiovascular Screening
For the woman who suspects she
is at risk, or for any woman who wishes to determine her specific risk,
we recommend this screening. This level includes a risk assessment
by a Cardiovascular Nurse; blood pressure, Body Mass Index, cholesterol
with glucose, Ankle Brachial Index, and EKG. Plus a gift to you from our
Boutique.
Level III
Cardiovascular Screening Plus
Recommended for the woman at
risk or for the woman who prefers a more complete assessment, this
screening builds upon Level II. In addition to the services provided in
Level II, add the following: abdominal screening, carotid and echo
imaging screening. And as a special treat, a chair massage and 2
gifts from out Boutique.
Level IV
Cardiovascular Screening Advanced
For the woman who knows she is
at risk or for the woman who desires a comprehensive assessment, we
recommend this screening. This screening level includes all of the
services provided in Levels II and III and will go one step further: the
patient will spend 30 minutes with a Board Certified Cardiologist to
discuss her personal plan for risk reduction. And then.. a chair
massage and 2 gifts from our Boutique.

Level V
Full Cardiology Consultation
Insurance/3rd Party Billing
This is recommended for women
identified to be at a high risk for cardiovascular disease, or for women
with known CV disease. Referral to this full consultation level
may be made at any of the prior levels or at the woman's request.
